A Subsidy Health and Safety inspection was conducted on 5/29/18 from11:00am to 2:00pm to review supplemental health and safety requirements. During the inspection there were 94 children ages four months old through ten years old in care with 14 staff. Children were observed participating in various activities in the classroom, eating lunch, and during rest period. Records were reviewed for 12 children and 13 staff/board members. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: VENDSUB-000-006-B-11 Description: Based on a review of the facility allergy list and 12 children's records, it was determined that the vendor did not ensure that a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: 1. The record for child #11 contained documentation from a physician that child #8 was allergic to eggs. However, there was no allergy plan present for child #8 at the facility. 2. The record for child #12 contained documentation from a physician that child #9 was allergic to eggs, nuts and seafood. However, there was no allergy plan present for child #9 at the facility. 3. Staff #10 reviewed the record for child #11 and child #12, and confirmed that neither child had an allergy plan at the facility. Plan of Correction: The vendor responded: The parents of child #11 and child #12 will be asked to provide an allergy plan that has been completed by the child's physician. All newly enrolled children with a diagnosed food allergy will have an allergy plan that has been completed by the child's physician prior to starting.
