An unannounced monitoring visit and facility tour was conducted from 2:50pm-4:15pm. At the time of inspectors' arrival there were 42 pre-school aged children in care with 5 staff members and 1 administrator. Children were observed participating in nap, snack,indoor and outdoor play, transitioning between activities, and toileting and hand washing. A sample of 5 children's records and 5 staff records were reviewed. Injury reports and medications were also reviewed. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.
Standard #: 22VAC40-185-130-A Description: Based on record review it was determined the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: The record for child #1(enrolled 5/1/18) did not contain documentation of immunizations. Child #1 was in care during this inspection. Plan of Correction: Center will request from parent at pick up.
Standard #: 22VAC40-185-60-A Description: Based on record review it was determined the center did not ensure each child's record contained all of the required components. Evidence: The record for child #1(enrolled 5/1/18) did not contain proof of identity and age, the address of one of the two required emergency contacts. Plan of Correction: Center will request information from parent at pick up.
Standard #: 22VAC40-185-560-G Description: Based on observation it was determined the center did not ensure when food is brought from home it is labeled with the child's name and date. Evidence: The lunchboxes for early childhood classrooms #1 and #3 did not contain dates and names. Plan of Correction: Center staff will ensure that each child's lunchbox is properly labeled.
