An unannounced monitoring inspection was conducted in the Center on May 17, 2018. As reported by staff, 52 children are currently enrolled in the program. The Licensing Inspector verified by observations and staff report that 49 children attended the center on the date of inspection. The LI observed the children in 4 classrooms. There were 11 staff observed in the classrooms supervising the children's activities. Two additional support/administrative staff were also present in the center. The LI reviewed a sample of 5 children's records. Medications were observed in the Center. Multiple staff have current training in medication administration (MAT). A discussion was held with the Monitoring and Compliance Supervisor regarding documentation of contacts with the applicable fire official. Please continue to follow up for an annual fire inspection. The Center's last fire inspection was completed on 05/09/17. The Center was maintained in good condition. Each classroom was supplied with age appropriate materials for the children's activities. Staff were observed interacting with the children. The findings were reviewed at exit. The inspection was conducted from 11:00am to 2:20pm. If you have any questions, please call 540-309-2310. Standard #: 22VAC40-185-140-A Description: Based on a sample review of children's records, the Center failed to ensure documentation of a physical examination by or under the direction of a physician was available for review for a child. Evidence: The LI reviewed a sample of 5 children's records. Documentation of a physical examination form was not available for review in one of five. No documentation of a PE for child #4 was available for review, the record noted the child's entry date was 11/20/17. Plan of Correction: The Family Development Specialist will contact the parent and request documentation of a current PE form. Documentation will be maintained in the child's record. Staff will audit records to ensure current documentation is complete for each child.
