The licensing inspector conducted an unannounced monitoring inspection Friday, June 1, 2018 from approximately 10:50 AM to 4:15PM. The following ratios were observed with a total of 96 children present during the inspection in the Preschool building. Younger Infants - 5 children with 2 staff Older Infants - 6 children with 2 staff Toddlers (Red Room) - 13 children with 3 staff Twos (Yellow Room) - 17 children with 3 staff Twos & Threes (Green Room) - 10 children with 2 staff Threes & Fours (Blue Room) - 18 children with 2 staff Fours and Five (Orange Room) - 5 children with 1 staff Fours and Five (Purple Room) - 22 children with 3 staff Get Well Place ? 0 children 1 nurse The licensing inspector observed 87 school age children present with 9 staff members during the inspection in the Village school age building. During the inspection, the licensing inspector observed children being held and fed, having their diapers changed, taking bathroom breaks and washing their hands, playing musical instruments, having books read to them, playing outside, sorting counting bears, painting, and eating lunch and snack. Lunch consisted of garden burgers, corn, peaches and milk. Snack consisted of cereal and milk. Staff was observed interacting with the children and providing assistance when needed. All areas of the facility in both the Preschool and School Age buildings including classrooms, hallways, bathrooms, kitchen, and playgrounds were inspected. The buildings were equipped with toys and supplies and items were available to the children. The following information was reviewed: emergency supplies, injury reports, fire and shelter-in-place drills, daily attendance, daily sheets, parent agreements, and staff training information. Medication is administered. Medication and authorization forms were reviewed. Transportation is provided and vehicles and transportation documents were inspected. Standard #: 22VAC40-185-130-A Description: Based on review of 10 children's records, one child began attending the center prior to immunization records being obtained. Evidence: During the inspection, the licensing inspector observed the record for Child #4 (date began attending 09/15/17) contained immunization records dated 10/06/17. Immunization records are required to be obtained before the child begins attending the center. Plan of Correction: We will make sure immunization records are obtained before a child attends the center.
Standard #: 22VAC40-185-60-A Description: Based on the review of 10 children's records, one record did not contain the required information. Evidence: During the inspection, the licensing inspector observed the record for Child #6 did not contain the address for one of the two emergency contacts listed in the record. Plan of Correction: Corrected during the inspection.
Standard #: 22VAC40-185-70-A Description: Based on the review of 10 staff records, two staff records did not contain the required information. Evidence: During the inspection, the licensing inspector observed the record for Staff #2 (hire date 05/05/18) did not contain the address for the staff's emergency contact. The record for Staff #9 (hire date 09/11/16) did not documentation of education required for a program leader. Staff #9 was identified as a program leader during the inspection. Plan of Correction: We will add the required information to the staff records.
Standard #: 22VAC40-185-290-3 Description: Based on observation, the center did not ensure all electrical outlets contained protective covers. Evidence: During the inspection, the licensing inspector observed an electrical outlet that did not contain protective covers beside the window in the Blue classroom. Plan of Correction: Corrected during the inspection.
Standard #: 22VAC40-185-520-B Description: Based on observation and interview, the center did not ensure children under nine years of age did not apply their own sunscreen. Evidence: During the inspection, the licensing inspector observed children arriving from local elementary schools, applying their own sunscreen and then playing outside. The children arriving from the local elementary school were as young as five years of age. Plan of Correction: We will make sure counselors apply sunscreen to children under nine years of age.
