Technical Assistance:
An exit meeting was completed by the licensing inspector with the program's director to review today's inspection findings. The following licensing standards were reviewed. 22 VAC 40-185-40. Operational responsibilities. E. The operational responsibilities of the licensee shall include, but not be limited to, ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department. 22 VAC 40-185-510. Medication. A. Prescription and nonprescription medication shall be given to a child: 1. According to the center's written medication policies; and

Comments:
The licensing inspectors conducted an unannounced monitoring inspection on Tuesday, May 29, 2018 from approximately 9:40 am to 2:30 pm to determine the center's compliance with licensing standards. The children were observed by the licensing inspector in the various classroom settings. The census for today's inspection consisted of 105 children and 16 staff. The licensing inspectors observed the children in care completing a variety of activities to include engaging in free play with age appropriate toys and their peers, participating in various centers, toileting, completing handwashing, and eating snack. There were eight children?s records and five staff's record reviewed during this inspection. The required posting and playground areas were inspected. Medication is being administered, medications, and written authorizations were reviewed for the center's compliance. Standard #: 22VAC40-185-130-A
Description: Based on review of children's record, the center did not ensure to obtain documentation of immunizations for each child. Evidence: During the inspection on May 29, 2018, Child#2's record (first date 9/05/17) was reviewed and there was no documentation of immunization found.

Plan of Correction: A letter was completed to submit to parents today to obtain a copy of immunization to the center or the child cannot return.

Standard #: 22VAC40-185-140-A
Description: Based on review of children's record, the center did not ensure to obtain a physical examination of each child. Evidence: During the inspection on May 29, 2018, a review of Child#2's record was completed and there was no documentation of a physical examination found.

Plan of Correction: A letter was completed to submit to parents today to obtain a copy of physical to the center or the child cannot return.

Standard #: 22VAC40-185-70-A
Description: Based on review of on one staff records, the center did not ensure all staff records requirements were completed. Evidence: During the inspection on May 29, 2018, a review of Staff#2 (date of hire 08/17) and Staff#4 (date of hire 08/28/17) were completed and the dates of contacts and the signature of person making the calls were missing for both references of each staff.

Plan of Correction: The Director will ensure all future references has date of contacts and signatures of person making calls.

Standard #: 22VAC40-185-260-A
Description: Based on review of fire inspection record, the center did not ensure an annual fire inspection was completed. Evidence: During the inspection on May 29, 2018, a review of the most recent fire inspection completed at the center was on 05/05/2017

Plan of Correction: The fire inspector was contacted today by the center's director and a request to schedule appointment was made.
