Comments:
A Subsidy Health and Safety inspection was conducted on on Tuesday, May 29, 2018 from approximately 9:40 am to 2:35 pm to review supplemental health and safety requirements. The children were observed by the licensing inspector in the various classroom settings. The census for today's inspection consisted of 105 children and 16 staff. The licensing inspectors observed the children in care completing a variety of activities to include engaging in free play with age appropriate toys and their peers, participating in various centers, toileting, completing handwashing, and eating snack. There were eight children?s records and five staff's record reviewed during this inspection. The required posting and playground areas were inspected. Medication is being administered, medications, and written authorizations were reviewed for the center's compliance. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Description: Based on review of children's record, the center did not ensure to obtain name, address, and phone number for each custodial parent's place of employment . Evidence: During the inspection on May 29, 2018, seven of eight children's records were missing the addresses of each custodial parent's place of employment.

Plan of Correction: A form will be sent home with all children requesting required information on May 30, 2018.
