Standard #: 22VAC40-185-70-A Description: Based on record review, staff records were missing required information. Evidence: 1 of 5 staff records reviewed were incomplete: Staff A was missing documentation of orientation and 2 of 2 required references. Plan of Correction: Staff has moved around a lot. Her record lost information in transit from ffacility to facility. We will locate documents to complete the file.
Standard #: 22VAC40-185-280-B Description: Based on observation, hazardous substances were not kept in a locked place. Evidence: 1) In the Pheonix Room, there was mineral based sunscreen labeled "keep out of reach of children" found unlocked on a shelf in the bathroom. 2) In the Toddlers Room, there was mineral based sunscreen labeled "keep out of reach of children" found unlocked on a counter top near the cubbies. Plan of Correction: We will lock the sunscreen in the cabinet.
