22 VAC 40-185-270 (building maintenance) and new background check regulations were discussed during the inspection. All staff must complete fingerprinting checks by September 30, 2018.
- Comments:

An unannounced monitoring inspection was conducted from 12:00pm until 3:30pm. At the time of the tour, there were 41 children in care with 6 staff present. 5 school-aged children with 1 staff person arrived at the center during the inspection. A sample of 5 children's records and 5 staff records were reviewed. Children were observed eating lunch, resting quietly during nap time and engaging in free play. Hand washing and restroom procedures were also observed. Medication, first aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills, transportation, children's injury reports and center postings were reviewed. Violations in the areas of physical plant and special care provisions and emergencies are identified on the violation notice and were discussed with the center director during the exit interview.

Standard #: 22VAC40-185-280-B Description: Based on observation, the center did not ensure that hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: There was a spray bottle of multi-surface cleaner and a spray bottle of sanitizer in an unlocked cabinet above the sink in the school-age classroom. The containers stated "caution", "keep out of reach of children" and "avoid contact with eyes." Plan of Correction: The spray bottles of sanitizer and multi-surface cleaner were placed in a locked cabinet during the inspection.
Standard #: 22VAC40-185-330-B Description: Based on playground observation, the center did not ensure that when playground equipment is provided, resilient surfacing shall comply with minimum safety standards. Evidence: The mulch surrounding the climbing equipment and slides on the playground measured between 1 inch to 4 inches in all areas, where 6 inches are required. Plan of Correction: Additional mulch was ordered today, 6/4/18, and will placed on the playground once the mulch is delivered.
Standard #: 22VAC40-185-500-B Description: Based on observation and interview, the center did not ensure that disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Evidence: 1. The foot operated trash can used to dispose of soiled disposable diapers in the two year old classroom was not working; therefore, staff have to lift the lid of the bin with their hands in order to dispose of soiled diapers. 2. Staff # 1 stated the trash can has been broken since August 2018 and that staff in the classroom did not alert management that a new one was needed. Plan of Correction: A new diaper bin will be purchased and placed in the classroom.
