An unannounced monitoring inspection was conducted from 11:00 a.m.-2:00 p.m. with the Outpost Director. 135 children, ages 4 months through 5 years old were under the care and supervision of 17 staff. Children were observed eating lunch, playing outside, working on an insect booklet, engaging in a computer activity, and forming letters. All playgrounds were inspected. Required information was posted. Both the fire and health inspections are current. The last emergency escape drill was 4/30/18. The first aid kit was complete. 7 medications, 13 staff records, and 12 children's records were reviewed. Standard #: 22VAC40-185-70-A Description: Based on review of 13 staff records, the center did not maintain complete staff records. Evidence: Staff A's two reference checks did not include the signature of the person making the call for references. Plan of Correction: This was corrected during the violation.
Standard #: 22VAC40-185-340-A Description: Based on review of seven medications, the center did not ensure the care and protection of children in care. Evidence: Child A's emergency medication for food allergies, Epipen Jr., expired in December 2017. Child B's emergency medication for asthma, Proair HFA inhaler, expired in April 2018. Plan of Correction: Both parents will be notified of this today.
Standard #: 22VAC40-185-450-C Description: Based on observation and staff interview, staff did not wash crib sheets daily. Evidence: The crib sheet for Child C was dirty. Staff indicated that the crib sheets are washed weekly not daily. Plan of Correction: Infant room staff will be reminded to wash linens daily. Child C's crib sheet will be replaced today.
Standard #: 22VAC40-185-510-G Description: Based on review of seven medications, the center did not have written authorization to administer prescription medication to children in care. Evidence: Child A's Epipen Jr., Child B's Proair HFA inhaler, Child D's inhaler, and Child E's inhaler did not have written authorization to administer the medication from a parent or guardian. Plan of Correction: All parents will be contacted regarding this. Two staff members will be checking the medications and forms.
