An unannounced monitoring inspection was conducted June 1, 2018 from 9:37am-12:00pm. There were 132 children (infants-5yrs.) directly supervised by 23 staff. The physical plant, 10 staff records, 10 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, spanish class, music class, technology class, show and share circle time, reading books, playing outside, practicing their letters and numbers. Diapering and hand-washing procedures were also observed. There was an abundant supply of materials for the children. The center was clean and organized. Interactions between children and teachers were observed and deemed to be positive. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-160-C Description: Based on record review, not all staff have submitted the results of a follow-up tuberculosis screening at least every two years from the date of the first initial screening. Evidence: The most recent Tuberculosis for staff # F was completed on 4/30/2016. Plan of Correction: Staff will be sent out to complete the TB.
Standard #: 22VAC40-185-270-A Description: Based on observation, equipment in the center was not kept in a safe condition. Evidence: In the Pre K classroom, a play kitchen is missing a piece of the toy and there is a small hole which may cause a finger entrapment. Plan of Correction: At the time of inspection, the hole was patched to prevent any potential injury.
Standard #: 22VAC40-191-60-C-2 Description: Based on review of ten staff records and interview with the center, the center did not obtain central registry report for one staff member. Evidence: Staff H does not have a completed central registry finding in the record. Plan of Correction: The background check investigation was sent out. We will follow up and document.
Standard #: 63.2-1720.1-B-3 Description: Based upon three of the ten staff records reviewed, out of state child abuse and neglect results were not obtained for all employees. Evidence: Three staff documented that they have lived out of the state of Virginia in the preceding 5 years, and no out of state child abuse and neglect results were obtained. (Staff C; DOH 7/05/2017; Staff D: DOH 5/21/2018; Staff J DOH2/26/20218). Plan of Correction: We will follow up on the out of state check that were sent out but results were not obtained. We will document the information.
