Comments:
An unannounced monitoring inspection was conducted today from 10:45am-4:00pm. There were 340 children (infants-5yrs.) directly supervised by 74 staff. The physical plant, 10 staff records, 10 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, dancing, singing, participating in circle time, playing outside, napping and eating lunch. Today's lunch consisted of breaded Pollock with french fries, baby carrots with ranch, oranges and milk. Diapering and hand-washing procedures were also observed. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice. Standard #: 22VAC40-185-130-B
Description: Based on record review, the center did not obtain documentation of additional immunizations once very six months for children under the age of two. Evidence: The file of child#1 has an immunization record that was last updated on 10/11/2017. The file of child#2 has an immunization record that was last updated on 09/28/2017.

Plan of Correction: We will re-request an updated record.

Standard #: 22VAC40-185-70-A
Description: Based on record review, not all staff records were complete. Evidence: Staff#2's record did not contain documentation of 2 required references checks.

Plan of Correction: We will complete the reference checks on that employee.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment of the center were not maintained in a safe and operable condition. Evidence: In the two's playground five out of seven tricycles were missing the rubber handle bar grip exposing a rusted metal handle. In the Sweet Peas classroom, the children's play sink is missing 3 knobs exposing small holes that could cause a finger entrapment. In the Bluebonnets' classroom the children's play sink is missing a knob exposing a small hole that could cause a finger entrapment. In the Zebras' classroom a sink that children use to wash their hands was observed missing a piece of laminate from the edge of the counter top exposing the pressed wood interior.

Plan of Correction: By the end of the week we will place an order to repair the tricycles. We will cover or close the holes by the end of the week. We will have facilities repair the counter edge by the end of next week.

Standard #: 22VAC40-185-280-B
Description: Based on observation, cleaning materials were not kept in a locked place using a locking method that prevents access by children. Evidence: In the Magnolia's classroom, cleaning materials were stored in an unlocked closet.

Plan of Correction: We will address it directly with the teachers of that classroom, and we will remind all teachers during the school's kick off meeting on August 2nd.

Standard #: 22VAC40-185-290-3
Description: Based on observations, not all unused electrical outlets had child protective covers. Evidence: In the two's playground, 2 outlets observed next to the entrance door without protective covers.

Plan of Correction: Facilities will replace the cover on that external outlet box by the end of next week.

Standard #: 22VAC40-185-510-C
Description: Based on review of medication, the center's policies when addressing outdated medication were not followed. Evidence: Child#3 has a medication on site with an expiration date of March 2018.

Plan of Correction: We will notify the parents that we need a replacement for the medication.

Standard #: 63.2-1720.1-B-3
Description: Based on staff record review, there is no documentation that child abuse and neglect checks were initiated for an employee that resided outside the state of Virginia within the last 5 years. Evidence: Staff#1 (DOH: 09/26/2017) has resided outside the state of Virginia within the last 5 years, their records did not have documentation that out of state child abuse and neglect results were obtained.

Plan of Correction: The teacher's last day at Open Arms is Friday June 8th.
