Comments:
An unannounced monitoring inspection was conducted today from 11:50am-1:45pm. There were 55 children (infants-5yrs.) directly supervised by 10 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed transitioning from lunch into naptime. Diapering and hand-washing procedures were also observed. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice. Description: Based on observation, two electrical outlets were observed without a child-resistant protective cover. Evidence: Two unused electrical outlets were observed outside the infant's classroom without a child-resistant protective cover.

Description: Based on observation, food brought from home was labeled with a name but did not have a date. Evidence: In the infant's room refrigerator, two containers of baby food were observed labeled with the child's name but without a date on them.
