An unannounced monitoring inspection was conducted on June 5, 2018. The director was present. In the Infant/Toddler/Preschool area there were 7 children with 2 staff. These included 2 infants, 2 two year old children, and 3 three year old children. The children were playing with blocks and toys. School age children were not present. Five children's records and 3 staff records were reviewed. Emergency drills had been documented. There were no medications being administered. Inspection began: 9:50 Inspection ended: 12:30
Standard #: 22VAC40-185-140-A Description: Based on a review of 5 children's records, the center did not have documentation of a physical exam for each child in care over 30 days. Evidence: There was no documentation on file of a physical exam for Child # 1 (in care since 6/11/16), Child # 3 (in care since 3/7/18), and Child # 4 (in care since 3/11/18). Plan of Correction: Parents will be notified today to provide the documentation.
Standard #: 22VAC40-185-60-A Description: Based on a review of 5 children's records, the center did not have documentation of child updates and confirmation of up to date information in the records. Evidence: The records for Child # 1 who began care 6/11/16, and Child # 5 who began care 5/1/17 did not include documentation of updates and/or confirmation of up to date information. Plan of Correction: This will be corrected by the end of today.
Standard #: 22VAC40-185-500-B Description: Based on observation and information obtained the center did not ensure that the diapering surface was cleaned with soap and at least room temperature water and sanitized after each use. Evidence: The diapering surface was being cleaned with disposable disinfectant wipes, and not being cleaned with soap and water and then a sanitizing solution. Plan of Correction: This has been corrected today.
