Comments:
A Subsidy Health and Safety inspection was conducted on June 5, 2018 to review supplemental health and safety requirements. The director was present. In the Infant/Toddler/Preschool area there were 7 children with 2 staff. These included 2 infants, 2 two year old children, and 3 three year old children. School age children were not present. Five children's records and 3 staff records were reviewed. Emergency drills had been documented. There were no medications being administered. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Inspection began: 9:50 Inspection ended: 12:30

Description: Based on a review of 5 children's records, the center did not have a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction., Evidence: The record for Child # 1, who had documentation on file of a food allergy, did not have an allergy action plan on file.

Plan of Correction: The parent will be notified today to provide plan within 48 hours.
