An unannounced renewal inspection was conducted on Wednesday, June 6, 2018 from approximately 2:00pm until 5:00pm. Present today were: K/1st : Twenty-five children with three staff 2nd grade: twelve children with one staff 3rd grade: thirteen children with one staff 4th, 5th and Middle School: eleven children with one staff Observations were made while the children were arriving, during snack time and while preparing for outside activity time. Children were completing homework assignments, working on the computers, and some were preparing to go to swim team practice. While outside, the children played basketball, and kick ball. All required emergency supplies were present. Eight staff records, eight children's records and medication for four children were inspected.
Standard #: 22VAC40-185-510-D Description: Based on review of children's medications present at the center, the center did not ensure all medications present had the appropriate signed permissions for the center to dispense the medication. Evidence: Out of four children's medications present at the center, there was not an authorization form to dispense the medication for Child #1. Plan of Correction: Per Center: Send medication home with the parent today and ask that it be sent back with updated and correct doctor's form.
Standard #: 22VAC40-185-510-N Description: Based on review of children's medications present at the center, the center did not ensure all medications present were picked up within 14 days after parent's authorization has expired or disposed of the medication. Evidence: Based on review of children's medications present at the center, the center did not ensure all medications present for Child 2, Child #3 and Child #4 were sent home or disposed of after the children no longer attended the center. Plan of Correction: Per Center: Remove medications within measures of standards and implement a check system to update and remove medications every week.
