Standard #: 22VAC40-185-550-D
Description: Based on a review of the facility's emergency drill log and interview, it was determined that the facility did not ensure that a minimum of two shelter-in-place practice drills per year for the most likely to occur scenarios were completed. Evidence: 1. There was documentation of only one shelter-in-place drill (7/3/17) being completed during 2017. 2. Staff #5 (Program Director) reviewed the emergency drill record, and confirmed that only one shelter-in-place drill was completed during 2017.

Plan of Correction: The facility responded: Going forward we will ensure that a minimum of two shelter-in-place practice drills per year for the most likely to occur scenarios.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of five staff records, it was determined that the facility did not deny continued employment of a staff who did not have a search of the central registry finding within 30 days of employment. Evidence: 1. The record for staff #2 (date of hire 4/30/18) did not contain documentation of a completed search of the central registry finding. 2. Staff #5 (Program Director) review the record for the staff #2, and confirmed that the search of the central registry finding has not been received.

Plan of Correction: The facility responded: A search of the central registry for staff #2 was completed and mailed. We will call and check the status of the search of the central registry for staff #2. All new staff will complete upon employment. If it is not received within 30 days of employment, we will call the Virginia Department of Social Services to check the status of the request.

Standard #: 63.2-1720.1-B-3
Description: Based on a review of five staff records, it was determined that the facility did not deny continued employment of a staff who did not have a search of the central registry finding within 30 days of employment from any other state in which the employee has resided in the preceding five years. Evidence: 1. The record for staff #2 (date of hire 4/30/18) did not contain documentation of a completed search of the central registry finding. 2. Staff #5 (Program Director) review the record for the staff #2, and confirmed that the search of the central registry finding from from the state in which staff #2 previously resided in has not been received.

Plan of Correction: The facility responded: A search of the central registry from from the state in which staff #2 previously resided in will be completed and mailed. All new staff will complete upon employment. If it is not received within 30 days of employment, we will call the appropriate agency to check the status of the request.
