Comments:
The Licensing Inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing office on 6/1/2018. Interviews were conducted with groups of children relating to the allegations of transportation safety. The information gathered during this investigation supports the allegations, so the complaint is determined to be valid. Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office within 5 business days of receipt. Complaint related: Yes
Description: Based on interview, the center did not ensure that during transportation of children that safety belts are used.
Evidence: When interviewing two groups of school age children, both groups of children confirmed that some children do not wear their seat belts on the bus.

Plan of Correction: We have spoken to all of the school age children that they must put and keep their seat belts on at all times on the van. The driver will double check with all the children before driving the van that everyone has their seat belts on. In the event that while driving a child takes their seat belt off, the driver will immediately stop until the child has put their seat belt back on. We have reviewed the van rules and regulations with all the children. We also have a seat belt infraction form that will be filled out in the event a child does not adhere to the rules. Infractions may lead to reduced activity time for summer campers.
