Discussion of an incident that occurred on Thursday, May 31, 2018 within the Mighty Penguins room.
The following inspection took place between the hours of 9 am through 12 pm noon. Today, I met with the staff of Kiddie Academy located in Leesburg, Virginia. This unannounced Non-mandated Monitoring Inspection (NMI) was conducted as a result of an incident reported by the center to the Division of Licensing. Standard #: 22VAC40-185-40-E Description: Based on review and interview, the center's activities and services were not maintained in compliance with the center's own policies and procedures that are required by these standards. Evidence - 1. On the date of inspection (06/07/2018), it was determined that Staff #1 failed to follow the center's own polices and procedures with regards to supervision and identifying where attending children were at all times. 2. Based on page 42 of the Kiddie Academy's Guide to Daily Procedures, it states ". . . It is each employee's responsibility to know how many children are in the group and their whereabouts at all times." Staff #1 stated that she failed to conduct the required head counts on May 31, 2018, when leaving her room, before entering a new area, and one additional time once in this new area. 3. Administrative staff produced signed documentation (by Staff #1) indicating that this training was given to Staff #1 on 11/28/2011, and again on 09/22/2017. These trainings addressed supervision, head counts, and procedures on field trips. 4. Child #1 was left unsupervised in the "Mighty Penguins" room while the rest of the group (6 preschool children) went to a different room to practice songs for the upcoming graduation. Plan of Correction: Staff #1 was placed on administrative leave, until all facts were gathered and investigation was completed, at which time Staff #1 was terminated from employment. Center takes a zero tolerance approach to such matters. All staff were retrained on center policy.
Standard #: 22VAC40-185-340-F Description: Based on review and interview, a child under 10 years of age was not always within actual sight and sound supervision. Evidence - 1. On the date of inspection (06/07/2018), it was determined that on Thursday, May 31, 2018, between 10:52 am to 11:20 am, Child #1 (4 years of age) was not within actual sight and sound supervision of staff while alone in the "Mighty Penguins" room. 2. Staff #1 stated (during an interview conducted on the date of inspection) that she failed to conduct a head count as the children left the room, another head count while the children were in the hallway, and a final head count when the children arrived at their destination across the hall in the "Frogs" room. 3. Staff #1 also failed to realize that Child #1 was not with the group as she moved the children to their assigned spots while practicing a song for the upcoming graduation while in the Frogs room. 4. When Staff #1 realized that Child #1 was missing (approximately 27 minutes later), a search was conducted. Staff #1 found Child #1 hiding back in the Mighty Penguins room behind a computer table, with the child awake and lying up against the wall. 5. After a review of the video tape, the total amount of time that Child #1 was unsupervised was 28 minutes. Plan of Correction: (Same as above.)
