Comments:
Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on June 5, 2018 from 11:00 a.m. through 2:30 p.m. There were seventy-seven children in the care of fourteen staff. Seven staff and seven children's records were reviewed and updated. No medication is being administered at this time. Areas of non-compliance may be found on the violation on the violation notice. Areas observed were: outside play, free choice activity, hand washing/bathroom procedures, lunch and nap. Standard #: 22VAC40-185-160-C
Description: Based on record review, the center failed to ensure one staff member had obtained an updated tuberculosis screening. Evidence: 1. Seven staff records were reviewed. 2. Staff # 2 did not have documentation of obtaining an updated tuberculosis screening. 3. The last documented tuberculosis screening for Staff #2 was dated 10/01/2015.

Plan of Correction: Staff will obtain updated tuberculosis screening. Documentation will be sent to the licensing inspector.

Standard #: 22VAC40-185-290-3
Description: Based on observation, the center failed to have a protective cover in one outlet. Evidence: 1. An outlet located under a table at the entrance did not have a protective cover. 2. Staff confirmed there was not a protective cover in the electrical outlet.

Plan of Correction: A protective covering will be placed in the electrical outlet.
