Comments:
Subsidy Health and Safety inspection was conducted on June 5, 2018 from 11:00 a.m. through 2:20 p.m. to review supplemental health and safety requirements. There were seventy-seven children in the care of fourteen staff. Seven staff and seven children's records were reviewed and updated. No medication is being administered at this time. Areas observed were: outside play, free choice activity, hand washing/bathroom procedures, lunch and nap. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: VENDSUB-000-009-2
Description: Based on record review, the center failed to notify to the department within two business days of a serious injury. Evidence: 1. Injury report, dated 05/11/2018, was reviewed for Child #7. 2. On May 11, 2018, at approxiamately 10:20 a.m., Child #7 was pushed by another child off the slide, on the outside playground, and sustained a cut on the top of his head. 3. The parents took Child #7 to the doctor on the evening of May 11, 2018 and the child received "glue" to close the cut on the top of his head. 4. Staff confirmed the department had not been notified.

Plan of Correction: Any serious injury will be reported to the department within 24 hours.

Standard #: VENDSUB-000-012-E-1
Description: Based on record review, the center failed to ensure all staff had current certification in CPR. Evidence: 1. Seven staff records were reviewed. 2. Staff # 5 did not have documentation of a current certification in CPR. 3. Staff #5's CPR documented had a renewal date of 03/22/2018.

Plan of Correction: Staff will obtain recertification in CPR. Documentation will be submitted to the licensing inspector.

Standard #: VENDSUB-000-012-E-2
Description: Based on record review, the center failed to ensure all staff had current certification in first aid. Evidence: 1. Seven staff records were reviewed. 2. Staff # 5 did not have documentation of a current certification in first aid. 3. Staff #5's first aid documented had a renewal date of 03/22/2018.

Plan of Correction: Staff will obtain recertification in CPR. Documentation will be submitted to the licensing inspector.
