Discussed with center management considering alternative ways to warm milk bottles.

An unannounced investigation was conducted on 05/24/2018 in response to a self reported incident of an infant burning her foot in the infant classroom on 05/16/2018. Staff interviews were conducted and observations were made in the classroom. Staff and children's records were reviewed. Staff training records and related incident report were also reviewed. Description: Based on observation and staff interviews, staff did not always ensure care, protection and guidance while supervising children. Evidence: 1. Staff # 1 stated that on 5/16/18, at approximately 11:50am, she had been holding Child # 1 as she was being fussy and Staff # 1 was trying to comfort her. Staff # 1 stated that she was holding Child # 1 on her left side and Child # 1 was facing forward. 2. The infant classroom uses a crock pot to warm infant milk bottles. The crock pot is placed on top of the small refrigerator which is located in the corner of the room. The crock pot has a stainless cover which gets hot as it is being used. The crock pot was placed out of reach of children. 3. Staff # 1 stated that she walked to the crock pot to get Child # 1's milk bottle, as it was time for her feeding. As she got closer, Child # 1 stretched her left leg forward and her left foot touched the hot crock pot. Staff # 1 stated that Child # 1 was crying at that point, but Staff # 1 could not determine if Child # 1 was crying because she was fussy or because she may have burnt her foot. 4. Staff # 1 stated that as soon as she sat down with Child # 1, she noticed Child # 1's foot was red. 5. Child # 1 burnt the side of her left foot when her foot came into contact with the crock pot. 5. Staff # 1 stated that she immediately called Staff # 2 for help. Staff # 2 stated that Staff # 1 applied a cool compress to the side of the foot. 6. Staff # 2 stated that a large sized blister had formed immediately. Staff # 2 was able to show the licensing inspector a picture of the blister. It appeared to be approximately 2 inches long sideways. 7. Staff # 2 stated that she contacted the parents who arrived within 30 minutes to pick up Child # 1.
