63.2 Facilities and Programs..
The inspector conducted a self-report inspection investigation on 4/11/18 and 4/26/18 based on a report this office received on 4/11/2018 from the center. Records including video were reviewed and staff was interviewed. Based on the evidence gathered, there was a violation resulting from the investigation. Areas of non-compliance are identified on the violation notice. Please call your inspector at 276-698-9981 with any questions.
Standard #: 22VAC40-185-340-A Description: Based on observation, interview and record review, staff did not ensure their care, protection, and guidance when supervising children. Evidence: 1. The inspector reviewed a video from 4/9/18 in which staff #1 rubbed child #1's hand with a magic eraser for 2 minutes. 2. The inspector reviewed an incident report dated 4/9/18 which documented that staff #1 got a magic eraser and ran it under water, then wiped pen marks off child #1's hand. 3. The inspector interviewed staff #1 on 4/11/18 who stated she had rubbed the child's hand with the magic eraser. The inspector observed a red, scabbed mark the size of a dime on the child's left hand on the top. Plan of Correction: The Head Start Director notified all staff at all the head start centers to not use magic erasers on children and to keep them locked with other hazardous substances. The quality assurance monitor will include monitoring potential hazardous substances to the checklist when monitoring. Staff #1 received a written reprimand and was reassigned to a different classroom, and lost the position of site director.
