Comments:
An unannounced monitoring inspection was conducted today from approximately 12pm-2:20pm. There were 42 children in care directly supervised by 15 staff. A sample of 5 children?s records, 6 staff records and 2 medications were reviewed. The children were observed during nap time. 1 new staff has been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the adminstrator. If you have any questions, contact Kimberly.Sawyer@dss.virginia.gov.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on observation, children's records were missing required information. Evidence: 5 of 5 children's records reviewed were incomplete. 1) Child A was missing the addresses of 2 of 2 emergency contacts, father's residential information, nickname of child and both the employment information of both parents. 2) Child B was missing the addresses of 2 of 2 emergency contacts, the employment information of both parents, nickname of child . 3) Child C was missing the addresses of 2 of 2 emergency contacts, father's residential information, nickname of child and both the employment information of both parents. 4) Child D was missing the addresses of 2 of 2 emergency contacts, father's residential information, nickname of child and both the employment information of both parents. 5) Child E Child B was missing the addresses of 2 of 2 emergency contacts, the employment information of both parents, nickname of child.

Plan of Correction: We have a new form that we will use next school year. Systems for child records including emergency forms records will be updated in August 2018. The emergency binder in the front office will include the emergency contacts, addresses, etc. and be updated bi annually. The child file by family service workers will include all other medical employment, etc.

Standard #: 22VAC40-185-240-C
Description: Based on record review and interview with administrator, staff who work directly with children have not annually completed at least 16 hours of staff development activities that shall be related to child safety and development and the function of the center. Evidence: The administrator stated that these staff are substitutes and work with children as needed. Staff A and B did not have any hours of child safety and development training for the year of 2017.

Plan of Correction: Systems for 16 hours of annual training for substitute teachers will be initiated on June 18th 2018 with an 8 hour mandatory training to include health and safety, social/emotional development, and nutrition education for young children. Long term system will be initiated August 2018 for the new program year.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances such as cleaning materials were not kept in a locked place. Evidence: In Room 1, dish soap labeled "keep out of reach of children" was on the counter near the sink.

Plan of Correction: The dish liquid will be locked.

Standard #: 22VAC40-185-420-E-3
Description: Based on record review, staff did not request at least annually parent confirmation that the required information in the child's record is up to date. Evidence: 1) Child D's record was last reviewed 04/11/2017. 2) Child E's record was last reviewed 04/13/2017.

Plan of Correction: We have a new form that we will use next school year. Standard #: 63.2-1720.1-B-3
Description: Based on record review, an out of state Central Registry Search was not initiated for staff that have lived outside of Virginia within the last 5 years. Evidence: Staff A has lived in Washington DC within the past 5 years and an out of state Central Registry Search has not been initiated.

Plan of Correction: I will follow up on this, I think the search was completed.
