22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted beginning at 10:40 a.m. and ending at 2:00 p.m .There were 56 children in care ranging in age from 4 months to 5 years of age with eleven staff providing direct supervision. Infants were observed sleeping, in play areas and being fed. Toddlers were observed during story time. Two year olds were observed during music and dance time. Three and four year olds were observed on the playground. The four and fives class was observed during story time. School age children were observed arriving during the inspection due to the half-day of public school. Six children's records and five staff records were reviewed. Standard #: 22VAC40-185-80-A Description: Based on review of documentation and interview with staff, attendance was not complete. Evidence: In the infant room, at approximately 11:00 a.m., there was one child marked present and there were six infants in care. Plan of Correction: The staff was reminded to complete attendance during the inspection. The staff members will be retrained on the importance of completing the attendance sheet.
Standard #: 22VAC40-185-280-B Description: Based on observation, not all hazardous substances were kept in a locked place. Evidence: In the toddler class, the key was observed in the lock to the unlocked cabinet that contained cleaning materials including Steramine brand sanitizer cleaner. In the PreK class, in the unlocked cabinet under the sink contained Great Value brand All Purpose with Bleach and Steramine brand sanitizer cleaner Plan of Correction: Cleaners were locked during the inspection. Staff will be reminded to keep all cleaners locked.
Standard #: 22VAC40-185-510-N Description: Based on review of documentation, medications were not returned or disposed of within 14 days of the expired medication authorization. Evidence: The authorization for Camilia teething relief for Child #1 expired on 4/24/2018. The authorization for children's ibuprofen for Child #2 expired on 4/27/2018. The authorization for the Epi Pen for Child #4 expired on 4/11/2018. Plan of Correction: The parents will be notified to pick up the medications or to complete a new authorization.
Standard #: 22VAC40-185-570-A Description: Based on observation, safety straps were not secured in an infant feeding table. Evidence: Two infants were observed sitting in a drop-in table with the safety straps not secured. Plan of Correction: Staff were reminded to use the safety straps at all times and this was corrected during the inspection.
Standard #: 22VAC40-185-570-E Description: Based on observation, not all infant bottles were labeled and dated as required. Evidence: Five bottles were observed in a basket in the fridge that did were not labeled with the child's full name and date. Plan of Correction: Staff were reminded to label and date the bottles. Bottles will be labeled and dated.
