Comments:
An unannounced monitoring inspection was conducted from 9:30 am - 12:30 pm. There were 68 children in care being supervised by 10 staff members. The children were observed participating in free play, circle time activities, crafts, and outdoor play. Lunch served today is hamburger on wheat bun, banana, corn and milk. Ten children's records and seven staff records were reviewed. Four medications were reviewed and in compliance. There are currently three staff members on site who are MAT (Medication Administration Training)certified. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on review of ten children's records it was determined that not all records were complete. Evidence: Child A with a start date of 09/05/2017 did not have documentation of proof of identity on file.

Plan of Correction: Administration will request proof of identity for child A.

Standard #: 22VAC40-185-340-F
Description: Based on observation and interview it was determined that not all children under 10 were within sight and sound supervision. Evidence: Child B (d.o.b. 11/12/11) was observed exiting the bathroom and walking to the playground without staff supervision.

Plan of Correction: Administration will retrain staff to ensure that all children are within sight and sound supervision at all times.

Standard #: 63.2-1720.1-B-2
Description: Based on review of seven staff records it was determined that not all staff members had results of eligible fingerprinting checks before the first date of employment. Evidence: Staff A with a start date of 06/11/2018 did not have eligible results of fingerprint checks on file.

Plan of Correction: Staff A will not return to work until eligible results of a fingerprint check has been received.
