An unannounced monitoring inspection was conducted in the Center on June 11, 2018. As reported by the Center Supervisor, there are 87 children currently enrolled in the program. At the time of the inspection, 74 children were present, the ages of the children ranged from infancy to 5 years. The children were grouped in 8 classrooms. The LI observed each group of children during varied activities which included: free play with blocks and legos, a teacher-directed activity (letter bingo), diapering and hand washing procedures, lunch, nap time, afternoon snack and dismissal. Staff were observed reading to children in a classroom; staff were observed interacting with the children in each classroom. A sample of 8 children's records was reviewed. Partial records were reviewed for four staff. Documentation of the annual fire and health inspections was current. Prescribed medications were observed in the center. Standard #: 22VAC40-185-60-A Description: Based on a sample review of children's records, the Center failed to ensure the following information was complete in each child's record: chronic physical problems and pertinent developmental information and any special accommodations needed. Evidence: Two of 8 records reviewed (children 7 and 8) did not address chronic physical problems and developmental information and any special accommodations needed. Plan of Correction: The Family Development Specialist will review the records with a parent of each child to ensure all information is addressed as applicable.
Standard #: 22VAC40-185-90--A Description: Based on a sample review of children's records, the Center failed to ensure a written agreement between the parent and the center was in each child's record by the first day of the child's attendance. The agreement shall be signed by the parent and include: 1. An authorization for emergency medical care should an emergency occur when the parent cannot be located immediately unless the parent states in writing an objection to the provision of such care on religious or other grounds; 2. A statement that the center will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible if so requested by the center; and 3. A statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately. Evidence: One of eight records reviewed by the LI did not contain a signed agreement between the parent and the center. The record for child #7 noted a legal change of custody in March, 2018. The written agreement in the record had not been updated to reflect the current custody arrangement. There was no signed, written agreement between the center and the legal guardian. Plan of Correction: The Family Development Specialist will review the information with the responsible party. A written agreement will be signed and maintained in the child's record. Information in records is monitored on-going.
