An unannounced monitoring inspection was conducted in the Center on June 12, 2018. There were 102 children present, the children were observed in 5 separate groups. The children's ages ranged from 18 months to 12 years. There were 16 staff members and the Program Director present during the inspection. The children were observed in their classrooms during the lunch meal, free play, nap/rest time, and movie time. Dismissal procedures were observed in the late afternoon. A sample of 8 children's and 5 staff's records was reviewed. Medications for several children were observed in the Center. Documentation of the Center's annual fire and health inspections was current. A discussion was held with the PD regarding Standard 22VAC40-185-550.H. and the document which identifies local emergency contact information, potential shelters, hospitals, etc. which is to be kept in the vehicles that centers use to transport children to and from the center. Please also include the center's complete physical address on the document. Findings of the inspection were reviewed at exit with the Program Director. Standard #: 22VAC40-185-540-C Description: Based on inspection of the first aid kit, the Center did not ensure all required items were supplied in the kit. Evidence: The LI inspected items in the first aid kit that is kept on the Center's bus, the kit contained only one triangular bandage. Plan of Correction: The Program Director will add another triangular bandage to the first aid kit. Each kit will contain at least two triangular bandages.
Standard #: 63.2-1720.1-B-2 Description: Based on a sample record review, the Center failed to ensure national fingerprint background checks were obtained on individuals prior to their dates of employment. Evidence: Two of five staff records reviewed (staff 3 and 4) indicated employment dates prior to obtaining criminal background checks. Dates of employment were noted as 01/25/18 and 01/29/18, national fingerprint background checks were verified on 02/09/18 and 02/07/18 respectively, Plan of Correction: The responsible staff will monitor dates to ensure criminal background checks are verified prior to an employee starting work in the center.
