Comments:
An unannounced monitoring inspection was conducted with the director beginning at 10 AM and ending at 3:20 PM. There were one hundred twenty-three children in care during the inspection. The children's ages ranged between three months and nine years old. The infant children were observed being bottle feed, eating lunch, and preparing for nap time and playing with soft toys. The older children were observed playing on the playground, doing arts and craft, playing with blocks and manipulatives. Ten children's records were reviewed. Six staff records were reviewed and in compliance. Five medications were reviewed and in compliance. The first aid kit, flashlight, and weather-band radio were observed and in compliance. The emergency escape drill log was reviewed and in compliance. The most recent escape drill was documented as 5/31/18. The most recent shelter-in-place drill was documented as 3/20/18. The most recent health inspection was documented as 4/20/18. The most recent fire inspection was 11/15/17. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of documentation, not all of the children's records were complete. Evidence: The records for Child A and Child B were missing the complete place of employment address for one parent.

Plan of Correction: The center will obtain the missing information from the parent and place it in the children's records.

Standard #: 22VAC40-185-270-A
Description: Based on observation not all outside areas and equipment of the center were maintained in a safe and operable condition. Evidence: On the school age children's playground there was a wheelbarrow with standing water and wood pieces accessible to children. On the preschool playground there was a gap between the resilient surfacing and sidewalk creating a tripping hazard. The gate between the preschool and infant playground had a v-shaped gap between the fence and post measuring 4.5 inches at the top narrowing to 2 inches at the bottom creating entanglement hazard. On the preschool playground there are six areas of the back fence that have a gap measuring more than 5 inches wide and 10 inches deep between the wooden boarder and fence creating an entanglement and tripping hazard.

Plan of Correction: The center emptied the wheelbarrow during the inspection. The wheelbarrow will turned over after use so that it will not become filled with rain water. The gap between the resilient surfacing and sidewalk was filled in with rubber mulch during the inspection. The center will fasten a 2X4 board to the gate where the v-shaped gap is located on the fence between the preschool and infant playground to resolve the hazard. The center will fasten the back fence to wooden boarder to eliminate the gap.
