Technical Assistance:
Inspector spoke with the Director about 22VAC40-185-550 and 22VAC40-185-510 during this inspection.

Comments:
An unannounced monitoring visit and facility tour was conducted from 9:58am- 10:45 am. At the time of inspectors' arrival there were 33 pre-school and school aged children in care with 7 staff members and 1 administrator. Children were observed participating in indoor play, outdoor play, transitioning between activities, and hand washing. A sample of 5 children's records and 5 staff on-site records were reviewed. Injury reports and medications were also reviewed. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined the center did not ensure that all areas inside and outside, were maintained in a safe and operable condition. Evidence: There were open boxes of gloves on top of the counter at child height in the Early Childhood Room 1 and 3 presenting potential choking/suffocation hazards.

Plan of Correction: Gloves were moved during this inspection.

Standard #: 22VAC40-185-290-3
Description: Based on observation it was determined that the center did not ensure that in areas utilized by preschool aged children all unused electrical outlets had protective covers. Evidence: In the Early Childhood Room #1 there was an uncapped electrical outlet by the TV within child reach. Preschool aged children were in care in this room during this inspection.

Plan of Correction: The outlet was capped during this inspection.

Standard #: 22VAC40-185-550-M
Description: Based on record review it was determined the center did not ensure that the written record of children's serious and minor injuries contained all of the required components. Evidence: Injury reports dated 4/23/18, 5/4/18, and 5/6/18 from Early Childhood Room #2 did not contain future action to prevent recurrence.

Plan of Correction: Staff will be retrained on completing Injury Reports and utilizing the updated form.

Standard #: 22VAC40-185-560-G
Description: Based on observation it was determined the center did not ensure that when food is brought from home the food container is clearly dated and labeled in a way that identifies the owner. Evidence: There were no dates on the lunches from Early Childhood Room #2 and 3.

Plan of Correction: Staff will be retrained on procedure for dating food brought from home.
