Comments:
An announced record review was conducted on June 14, 2018. The licensing inspector reviewed Administration, Background Checks and Licensure and Registration Procedures. The sample size consisted of six staff records. Areas of noncompliance are identified on the Violation Notice.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on record review, in two of six staff records reviewed, the licensee did not ensure that each staff record contained all required information. Evidence: During the inspection conducted on June 14, 2018, there was no documentation of orientation training in the record for Staff #1 and Staff #4.

Plan of Correction: We will ensure that orientation is done the day of hire for all employees.
