Comments:
An announced record review was conducted on June 14, 2018. The licensing inspector reviewed Administration, Staff Qualifications and Training, Background Checks and Licensure and Registration Procedures. The sample size consisted of seven staff records. Areas of noncompliance are identified on the Violation Notice.

Violations:
Standard #: 22VAC40-185-240-C
Description: Based on record review, in one of six staff records reviewed, the licensee did not ensure that program directors and staff who work directly with children shall annually attend 16 hours of staff development activities that shall be related to child safety and development and the function of the center. Evidence: During the inspection conducted on June 14, 2018, there was documentation of four hours of annual training obtained between June 2017-May 2018 for Staff #3.

Plan of Correction: Going forward we will ensure that all employees have all training even if they are out on leave.

Standard #: 63.2-1720.1-B-3
Description: Based on record review, in one of seven staff records reviewed, the licensee did not obtain a copy of the results of a search of the central registry maintained pursuant to ? 63.2-1515 and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years for any founded complaint of child abuse or neglect against him. Evidence: During the inspection conducted on June 13, 2018, a Sworn Statement or Affirmation form for Staff #7 (date of hire 11/01/17) indicated that the staff member has lived outside the State of Virginia within the last five years and the center had not obtained an Out of State Child Abuse and Neglect Search Request for that state.

Plan of Correction: We will give the staff a new sworn disclosure statement and run the staff member's CPS check from Florida.
