21 Children were in care during the inspection, Children were observed during rest time free play and outside time. The age range of the children was seven months to five years old. No children were on medications and two staff were current in MAT. Inspection time 11:30 - 2:30pm
Standard #: 22VAC40-185-210-A-1 Description: Based on review not all staff were qualified to be program leader. Evidence: Staff A and B did were working in the position of program leader and the files did not include documentation of 24 hours of lead teacher training and did of include documentation of education. Plan of Correction: I will give the staff the 24 hours of training and have staff that meet program leader quifations supervising.
Standard #: 22VAC40-191-60-C-1 Description: Based on review of six staff records , the center did not have an original criminal history record report with in 30 days of employment for staff, Evidence: Staff C hire date of 9/11/17 was not in the record. Plan of Correction: All staff backgrounds checks are current.
Standard #: 22VAC40-191-60-C-2 Description: Based on review of six staff records, the center did not have central registry reports with in 30 days of employment for staff. Evidence: Staff C with a hire date of 9/11/17 did not have a returned, central registry report. Plan of Correction: All CPS check have been re sent and returned
