Standard #: VENDSUB-000-006-B-11 Description: Based on a review of children's records on 06/14/2018, the center did not ensure that a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction was obtained. EVIDENCE: The record of Child #7, who has a food allergy, did not contain a written allergy care plan. Plan of Correction: Will give parent an allergy action plan form today, 06/14/2018. Will make sure that the completed form is returned by tomorrow.
