We discussed the requirements for staffing the school age program, immunization reports and hand washing for children. The risk assessments for violations were provided. Also, we talked about the record requirement for diaper ointment applications, and your playground maintenance plans and procedures. Also, we talked about appropriate techniques for redirecting infants who want the same toy or equipment.
Thank you for your assistance during today's unannounced monitoring inspection conducted from 9:55 AM to 4:30 PM. Today, there were ninety children, ages infancy through twelve years, with thirteen staff. I viewed program activities, daily routines, staffing, supervision, interactions with children, equipment, posted information, medication, injury records, emergency supplies and drill records, agency reports, infant care and records, diapering procedures, nutrition and meals/snacks, sunscreen, diaper ointment, ten records for children, ten staff records, staff qualifications, staff training, transportation, indoor and outdoor areas. The children were provided a variety of group activities including art, stories and discussions. Outdoor activities have included water play with sprinklers. Standard #: 22VAC40-185-130-A Description: Based on a review of ten records for children, and an interview with administrative staff, the center did not obtain documentation of immunizations before child 3 began attending the center. Evidence: The first date of attendance was April 3, 2018 and the immunization report in the file was completed by the health care provider on May 1, 2018 and the FAX date was May 3, 2018. Plan of Correction: Administrative staff who enroll children will inform the parents that children may not begin attending the center until an immunization record is provided to the center. The director and assigned administrative staff who accept paperwork for enrollment will check to make sure all required information was submitted before allowing children to begin.
Standard #: 22VAC40-185-350-C Description: Based on observation of the children and staff in the school age room,and interviews with staff, the required staff-to-children ratio applicable to the youngest children in the mixed age group of children ages five through twelve years , was not complied with during a period of the morning. The required ratio was one staff for every eighteen school age children. Evidence: At 10:05 AM thirty-nine school age children were seen with two staff. When questioned, staff indicated that three children arrived at 9:40 AM and had not attended the previous week. Staff indicated that the third staff person was not scheduled until 10:45 AM. Administrative staff indicated that an extra staff person was in the building with another group and the school age staff did not ask for assistance when the additional staff arrived. Staff reported that eighteen children in the group were ages five through eight. Plan of Correction: The director will address the need for compliance with staffing ratios with the lead staff in the school age room. The teachers will be told to conduct head counts during the morning and to communicate when they are at their limit for the staff present. The staff will be directed to contact the director or assistant director immediately when unexpected arrivals occur and additional staff is needed immediately. Administrative staff will plan staffing for weeks when higher attendance of children is anticipated so that children have staff present to meet ratio requirements.
Standard #: 22VAC40-185-500-A Description: Based on observation of diapering procedures, and an interview with staff, the staff person did not have a two year old child wash hands after a diaper change. Evidence: During observation of a diaper change in the two year old room, after a diaper change the staff person had the child rejoin the group at the rug and she went to wash hands at the sink without the child. Plan of Correction: The director will review the required hand washing procedures, including having children was hands after each diaper change, with the staff in the two year old room. Administrative staff will conduct observations of diapering procedures in each room to make sure all safe and healthy procedures are followed.
