The licensing inspector conducted an announced non-mandated monitoring inspection regarding a self-reported incident. Video footage of the reported incident, staff interviews, and review of documentation occurred today, 6/18/18. Based on the evidence gathered there was a violation cited, as non-compliance with a standard did occur on 5/30/18 as reported. Please contact me if you need further assistance. Standard #: 22VAC40-185-410-1 Description: Based on review of saved video, discussions with staff, and the self-reported information from the program, the facility failed to maintain compliance with this standard as required relating to rough handling of children. Evidence: Saved video of the incident showed staff #1 roughly handle child #1 on 5/30/18 by jerking the child up off of the cot by her upper arm and moving her to a standing position, approximately 1-1.5 feet away from the cot, onto the floor during rest time. The video showed that the childs feet were not touching the floor until staff #1put the child down, to a standing position. Staff interviewed also stated that they witnessed this occur, as depicted in the video, with staff #1 stating that "child #1 needs to leave the room because she isn't dealing with this today". Staff interviewed stated that child #1 did cry harder when she was jerked off of the cot by staff #1, and that once the child was placed down the child touched her upper arm and stated "ow my arm" while crying. Staff reported that they did not see any marks upon viewing of the childs arm, nor did the child complain or favor her arm after this incident. Staff reported that no medical treatment was obtained by the center or parents. Plan of Correction: Staff #1 was terminated on 5/31/18. All staff will be attending training on "Sight & Sound Supervision; Behavior Management" in July 2018. All staff will sign HoneyTree Rules of Conduct and Zero Tolerance Policy during the July meeting.
