The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on June 14, 2018 regarding allegations of physical plant concerns, ratios, injury reports and reporting, supervision and record keeping. The inspection was completed on June 14, 2018. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation supports part of the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details. Standard #: 22VAC40-185-550-M Complaint related: Yes Description: Based on a review of injury reports on 06/14/2018, the center did not ensure that the record of injuries contained the required information. EVIDENCE: Out of a sample of five injury records reviewed: three did not have documentation of future action to prevent recurrence, three did not have the date and time the parents were notified, and three did not have a staff and parent signature or two staff signatures. Plan of Correction: Will make sure closing supervisor will check forms for completion at the end of each night.
