Technical assistance was provided in the following areas of the standards: 22VAC40-191 (Background checks); 22VAC40-185-(2)-70-A (Staff records)
An unannounced monitoring inspection was conducted on 7/2/18 from 9:30am - 10:45am. At the time of entrance there were 39 school age children in care with five staff. Children were observed participating in various activities in the classrooms. Records were reviewed for five children and five staff during the inspection. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.
Standard #: 22VAC40-185-70-A Description: Based on a review of five staff records and interviews, it was determined that the facility did not ensure that a staff record is kept for each person with all of the required information. Evidence: 1. The record for staff #1 did not include documentation that two or more references as to character and reputation as well as competency were checked before employment. 2. Staff #6 (Program Director) confirmed that there was no documentation that two or more references as to character and reputation as well as competency were checked before employment. Plan of Correction: The facility responded: We will try and contact our corporate office to obtain them. However if unsuccessful, we will complete new references. All new employees will have two or more references as to character and reputation as well as competency completed before employment.
Standard #: 22VAC40-191-60-C-2 Description: Based on a review of five staff records, it was determined that the facility did not deny continued employment of a staff who did not have a search of the central registry finding within 30 days of employment. Evidence: 1. The record for staff #1 (date of hire 3/26/18) did not contain documentation of a completed search of the central registry finding. 2. Staff #6 (Program Director) reviewed the record for the staff #1, and confirmed that the search of the central registry finding has not been received. Plan of Correction: The facility responded: A search of the central registry for staff #1 will be completed and mailed. All new staff will complete upon employment. If it is not received within 30 days of employment, we will call the Virginia Department of Social Services to check the status of the request.
