Comments:
The licensing inspector conducted an unannounced complaint inspection on June 5, 2018. This inspection was conducted in response to a complaint that was received by the Valley Licensing Office on 05/23/2018, 05/25/2018. Interviews were conducted with staff relating to the allegations regarding care and protection of children and building maintenance. The information gathered during the investigation supports part of the allegations, so the complaint is determined to be valid. There were three additional violations resulting from this complaint investigation that was not related to the complaint. Complaint related: No
Description: Based on observation, the center failed to operate in a clean and safe condition. Evidence: The electrical outlet located on the back wall of the infant room was secured around the edges with peeling tape. 2. The outlet was in reach of crawling and walking children. 3. A large rug in the multipurpose room was torn and frayed on the edges posing a tripping hazard and discolored due to visible dirt and grime. 4. The multipurpose room is used by children. 5. A trap containing dead roaches was observed in the infant room. 6. Staff interviewed stated, "Two weeks ago, the center had sprayed during the weekend." When the staff arrived at work, she "swept up 60 dead roaches? in the infant classroom before the children arrived. 7. Staff interviewed said there had been live roaches in the classroom and staff would catch them in a cup and dispose of them. 8. Staff interviewed said live roaches had been found on the children's nap blankets. 9. Staff interviewed stated they would shake all blankets before giving them to children for nap. 10. Staff interviewed said a parent was changing his child's diaper and found a live roach on the stack of diapers. 11. One infant was observed crawling on the area rug.

Plan of Correction: The outlet will be secured. The large rug was cleaned and fray was removed.

Complaint related: No
Description: Based on observation, the center failed to protect children from hazards on the outside infant playground. Evidence: 1. The outside playground area, used by infants, had indoor/outdoor carpeting installed over the concrete. 2. The indoor/outdoor carpeting was not fastened and secured to the concrete and was loose and "bunched" in the areas where the children would crawl posing a tripping hazard. 3. Staff confirmed the indoor/outdoor carpeting was not fastened and secured to the concrete.

Plan of Correction: The carpeting will be secured.

Complaint related: No
Description: Based on observation and discussion with the staff, the center failed to provide enough cribs for the children in care. Evidence: 1. Eleven children were observed in the infant room. 2. Two of the eleven children were over 12 months of age. 3. There were only five cribs in the infant room. 4. Staff confirmed there were not enough cribs for the children in care.

Plan of Correction: More cribs have been added to the infant room.

Description: Based on observation and conversation with the staff, the center failed to label cribs for use by a specific child. Evidence: 1. Five cribs were observed in the infant room. 2. The cribs were not labeled for specific use by a child. 3. Staff confirmed the cribs were not labeled. 4. Nine of the eleven children observed in care were under the age of 12 months requiring a crib.

Plan of Correction: The cribs are labeled . Staff will make sure all cribs are labeled.

Description: Based on observation the center failed to have a working sink in the infant room and a non-absorbent surface on two changing tables. Evidence: 1. The portable sink in the infant room was not in operation. 2. Staff interviewed said it had not been working since 06/01/2018. 3. Staff interviewed said hand washing was being done down the hallway in the multipurpose room bathrooms. 4. The pads on the changing tables in the infant room and toddler classroom were torn and had been taped. 5. Staff confirmed the changing table pads were torn and had been taped.

Plan of Correction: A part for the sink was ordered and replaced. Sink is working. Changing tables have been replaced with commercial grade tables which do not require changing pads.
