Comments:
An unannounced monitoring inspection was conducted today. All groups are in ratio during the inspection The playground was not in use today due to inclement weather (Heat). 6 staff records were reviewed for specific items only. Full staff files are reviewed at another time at the main office. 10 children's records were reviewed. See violation notice for missing information. Emergency evacuation drills, Shelter-in-place drills, and medications are all in compliance. Additional space used for Summer SACC was measured today. The license will be modified to reflect the newly requested capacity. A Subsidy Health and Safety Inspection (SHSI) was also conducted today. LI was present today from 11:30 AM - 1:50 PM. Violations:
Standard #: 22VAC40-185-130-A
Description: Based upon a review of records, one child's record does not contain documentation of an immunization record. Evidence: Child #1's file does not contain a record of immunization.

Plan of Correction: I will contact the parent today to have the school fax the record to us.

Standard #: 22VAC40-185-280-B
Description: Based upon direct observation, hazardous materials were not stored locked. Evidence: Aerosol spray paints and cleaners labeled "Keep out of Reach of Children" were in an unlocked cabinet in the game room.

Plan of Correction: They were moved during the inspection. Staff will be reminded to store hazardous materials in locked cabinets.

Standard #: 22VAC40-185-540-A
Description: Based upon a review of first aid supplies, the first aid kit is incomplete. Evidence: Missing items include: 1 triangular bandage.

Plan of Correction: I will purchase a triangular bandage.

Standard #: 22VAC40-185-550-M
Description: Based upon a review of injury reports, reports do not contain all required information, Evidence: 3 of 4 reports did not document the method of notification to parents, or the time of notification.

Plan of Correction: I will make sure staff complete the forms correctly. I will remind staff to fill out every single line.
