Comments:
An unannounced complaint investigation was conducted with center director. Based on review of the injury report and staff interview, the allegations made by the complaintant regarding the timely reporting of a child's injury was determined to be valid. Description: Based on staff interviews and documentation, the facility failed to notify the parent immediately when a child sustains a serious injury. Evidence: on May 8, 2018, at approximately 10:55 a.m., a child fell and bumped his head on the playground. The parent was not notified by phone until 2:40 p.m.

Plan of Correction: Parent was notified of the injury via phone at 2:40 p.m. on the day of the incident. Staff have been retrained on the proper procedure regarding the proper procedure regarding parental notification and the time limits involved.
