Technical Assistance:
Standard 22 VAC 40-185-60 (children's records) as well as background check procedures were discussed with the center director during the inspection. All staff must complete fingerprint checks by September 30, 2018.

Comments:
An unannounced monitoring inspection was conducted from 12:15pm until 3:50pm. At the time of the tour, there were 62 children in care with 8 staff present. A sample of 6 children's records and 6 staff records were reviewed. Children were observed resting quietly during nap time, eating snack and participating in afternoon activities. Infant diapering, hand washing and restroom procedures were also observed. Medication, transportation, first aid and emergency supplies, documentation of emergency practice drills, children's injury reports and center postings were reviewed. Two violations in the areas of physical plant and staffing and supervision are identified on the violation notice and were discussed with the center director during the exit interview.

Standard #: 22VAC40-185-330-B
Description: Based on playground observation and measurement, the center did not ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards. Evidence: 1. The mulch at the base of three out of four slides on the pre-k/school-aged playground measures approximately 1 inch, where 6 inches are required. 2. The center director confirmed that the mulch does not measure 6 inches at the base of the slides on the pre-k/school-aged playground.

Plan of Correction: The center responded with the following: A work order for mulch was input on 7/3/18. Awaiting receipt from landscaping company. It will be completed as soon as possible.

Standard #: 22VAC40-185-350-D
Description: Based on observation and interview, the center did not ensure that during the designated rest period, the ratio of staff to children may be double the number of children to each staff required by subdivisions E 2 through 4 and 7 of this section. Evidence: 1. During nap time in the pre-k classroom there were 21 children, ages 3-5 years old, napping with 1 staff present. The number and ages of the children require two staff to be present. 2. The center director confirmed that the number and ages of children in the room require two staff to be present.

Plan of Correction: The center responded with the following: The classroom was in ratio all day, unfortunately, at rest time the teacher received another student and did not make management aware. The one child over ratio was corrected in that moment to be compliant with ratio.
