Inspector discussed updated background check regulations and documentation with the Director during this inspection.
An unannounced monitoring visit and facility tour was conducted from 11:11 am - 1:45pm. At the time of inspectors' arrival there were 21 infant-preschool aged children in care with 6 staff members. Children were observed participating in indoor play, lunch, transitioning between activities, toileting and hand washing, and nap. A sample of 5 children's records and 5 staff records were reviewed. Medications and injury reports were also reviewed. Areas of noncompliance are detailed in the Violation notice and were discussed with the staff in charge at the conclusion of this inspection.
Standard #: 22VAC40-185-130-A Description: Based on record review it was determined that the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: The record for child #1(enrolled 6/18/18) did not contain documentation of immunizations. Child #1 was in care during this inspection. Plan of Correction: Director will request immunization record from parent at pick up.
Standard #: 22VAC40-185-270-A Description: Based on observation it was determined that the center did not ensure all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: 1. In the Penguin class(infants) there was peeling paint on the wall and cubbies. 2. In the Otter class(toddlers) there was: -an unstable red shelf that rocked when pushed on that needed to be anchored -a 1/4" gap in the wall around a PVC pipe -peeling vinyl flooring by the back door -fungal growths in the mulch on the outdoor playground -peeling paint and rust on the exterior door 3. In the Jellyfish room(2-5yrs) there was: -safety latch on the drawer by the small corner sink did not work, there were large adult scissors, permanent markers, and batteries in the drawer -peeling paint and chipped plaster on the corners and walls -1.5" bolt on the shelf with the doll house -grime buildup on shelves and toys 4. In the Starfish class(2-3's) there was: -broken plastic edging on the orange shelf with sharp edges -peeling paint and chipped plaster mostly baseboard level -grime buildup on shelves and toys 5. All of these areas were in use and accessible to children during this inspection. The above noted items and areas presented potential injury hazards to the children in care. Plan of Correction: All maintenance will be completed by 8/3/18.
Standard #: 22VAC40-185-290-3 Description: Based on observation it as determined the center did not ensure all unused electrical outlets had protective covers in areas utilized by preschool aged children. Evidence: There was an uncapped electrical outlet in the kitchen play area in the Jellyfish class(2-5 year olds) during this inspection. There were preschool aged children in this room during this inspection. Plan of Correction: The outlet was capped during this inspection.
