Comments:
The licensing inspector conducted an unannounced monitoring inspection on 07/10/2018 from 10:12am to 2:05pm. The children were observed participating in circle time, identifying letters, singing nursery rhymes, listening to story time and eating lunch. A formal observation was completed with the Ladybugs and Butterflies #1 classrooms. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today. Five children?s records and five employee records were reviewed during this inspection. Medication is not being administered. Ten injury reports were reviewed as well as the center?s injury prevention plan, which was updated 10/2017. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 06/25/2018 Last shelter-in-place drill: 02/16/2018 Last fire inspection: 05/23/2018 Last health inspection: 08/11/2018 Today, the following child to staff ratios were observed: Ladybugs (16 months - 30 months) - 7:2 Caterpillars (Twos and Threes) - 9:2 Butterflies (Threes and Fours) ? 13:2 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-240-A
Description: Based on a record review, the facility did not ensure that one of five staff records contained documentation of 16 hours of annual training. Evidence: The record of staff person # 1(start date: 5/30/2017) contained only 4 hours of training for the period May 30, 2017 to May 29, 2018.

Plan of Correction: Staff #1 will begin to work on annual training hours on Friday 13th. She will have them done by Tuesday the 17th.

Standard #: 22VAC40-185-290-3
Description: Based on observation, the center did not ensure that electrical outlets had protective covers. Evidence: The butterflies #2 classroom had an uncovered electrical outlet located about four feet from the floor within the reach of children. There was also a surge protector with three uncovered outlets in the butterflies #2 classroom, the surge protector was out of the reach of children.

Plan of Correction: Corrected during the inspection.

Standard #: 22VAC40-185-330-B
Description: Based on inspection of the playground, the center did not ensure climbing equipment and swings had sufficient resilient material located in the fall zones.
Evidence:
The large climbing piece of equipment and the ground under both swing set seats has about 3 - 4.5 inches of resilient material. Both swing sets require 6 inches of resilient material underneath the swing set seats and in the fall zones. The large climbing piece of equipment requires six inches of resilient material in the fall zones.

Plan of Correction: We will call today and put in an order for mulch. Mulch will be installed by next Monday or Tuesday.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, the center did not ensure that two of five staff records reviewed had central registry findings within 30 days of employment. Evidence: Staff #2 (hire date: 10/21/16) had documentation of central registry findings dated 1/29/17. Staff #5 (hire date: 4/4/16) had documentation of central registry findings dated 5/18/16.

Plan of Correction: We will send out central registry findings earlier and we will make sure to follow up within 30 days.
