An unannounced monitoring inspection was conducted on July 11, 2018. Licensing Inspector arrived at 12:40pm and departed at 3:30pm. At the time of the tour there were 30 school age children present with three staff members during the summer camp program. Children were observed playing in various stations created in the cafeteria to include jump rope, dramatic play and blocks. The children were also observed working in small groups constructing houses. The sample size consisted of five children's records. Five medications were reviewed.

Description: Based on record review, in one of five children's records reviewed, the licensee did not ensure that all required information was maintained in each child's record. Evidence: During the inspection conducted on July 11, 2018, there was not a complete home address for one parent in the record for Child #3.

Plan of Correction: The parent corrected the file during the inspection.

Description: Based on observation and inspection of the facility, the licensee did not ensure that hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: During the inspection conducted on July 11, 2018, the following unlocked hazardous substances, labeled keep out of reach of children and warning, were observed in the facility: Boys Bathroom: disinfecting spray on the shelf Girls Bathroom: disinfecting spray and bleach-water solution on the shelf

Plan of Correction: Summer Camp Director will have a meeting with all staff about making sure that hazardous substances remain locked. The items were removed and locked during the inspection.
