Technical Assistance:
Technical Assistance was provided regarding raking the mulch to spread it for an more even surface.

Comments:
An unannounced monitoring inspection was conducted today from 9:00am-12:45pm. There were 37 children (infants-5yrs.) supervised by 10 direct care staff. The physical plant, staff records, children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing outside, and playing at the pool. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Positive interactions between staff and children was observed. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Standard #: 22VAC40-185-130-B
Description: Based on review of children?s records, the provider did not obtain documentation of updated immunizations once every six months for children under the age of two years. Evidence: Child #2 (dob 6/9/17) did not obtain an updated immunization record every six months, last record on file, 10/12/17.

Plan of Correction: I will email parent today and get an updated record.

Standard #: 22VAC40-185-270-A
Description: Based on observation the center failed to maintain the center in a clean, safe and operable condition. Evidence: 1. Children's furniture located in the housekeeping area of the preschool room is peeling and can easily be removed by the children. 2. Peeling paint was located on the window sills in the Pre K and Preschool classrooms.

Plan of Correction: I will contact the office and have them sand and re-paint the window sills.

Standard #: 22VAC40-185-510-E
Description: Based on review of medications, the center failed to label all medications. Evidence: Child #1 had medication on site without a label with the child's name, name of medication, dosage amount and time for administration.

Plan of Correction: I will contact a parent to get a label for the medication.

Standard #: 63.2-1720.1-B-3
Description: Based on review of staff records, the center did not have a completed out of state child abuse and neglect registry statement for each staff prior to date of hire. Evidence: Staff #1 does not have an out state child abuse and neglect registry statement on file before first day of hire.

Plan of Correction: I will send off the out of state paperwork today.
