22VAC40-191 Background Checks (22VAC40-191)
An unannounced non mandated inspection was conducted today between 9:40am to 11:20am. The purpose of this inspection was to assess previous violations which have been corrected. There were 15 children in care of 5 staff members. The children were observed getting ready for water play, doing circle time and getting diapers changed. 4 new staff records were reviewed. Standard #: 22VAC40-185-160-A Description: Based on record review, center did not obtain documentation of a negative tuberculosis (TB) screening for staff within 21 days of employment. Evidence: 1. Staff # 1 was hired on 5/14/18. 2. Staff # 1's record did not have any documentation of a negative TB screening in her file within 21 days of employment. Plan of Correction: We will obtain the results and place in staff file. Staff has already completed the TB screening.
Standard #: 22VAC40-185-500-A Description: Based on observation, children's hands were not washed with soap and running water after diaper changing. Evidence: 1. Staff # 2 was observed changing a child's diaper. 2. After the diaper change was complete Staff # 2 used a diaper wipe to clean the child's hands, instead of washing hands with soap and running water. Plan of Correction: Staff was re-trained during the inspection to make sure hand washing is done after all diaper changes.
Standard #: 22VAC40-191-60-C-2 Description: Based on record review and staff interview, center did not obtain a central registry finding within 30 days of employment for staff. Evidence: 1. Staff # 3 stated that Staff # 2 had been employed with the center in 12/2015 and had left employment in April 2017. Staff # 2 had been separated from the center for over one year. 2. Staff # 2 was re-hired on 5/14/18. 3. Staff # 3 stated that she used the central registry results from the last time it was completed, dated 4/18/16 thinking it would still be good for five years. 4. Central registry results were not submitted and obtained for Staff # 2 within 30 days of employment when staff # 3 was re-hired on 5/14/18. Plan of Correction: We will submit the central registry for this staff person as soon as possible.
