Discussed with center management background checks and Tuberculosis Screenings.
An unannounced complaint investigation was conducted today between 10:30am to 12:40pm. The inspection was conducted in response to a complaint that was received by the licensing office on 7/6/18. The complaint was related to a child getting bit on two different occasions, and that an incident report was not provided in a timely manner. The complaint was also related to the preschool classroom being over ratio. Interviews were conducted with staff. Staff and children records were reviewed. The information gathered during this investigation does not support the allegations, so the complaint is determined to be not valid. Standard #: 22VAC40-191-60-C-1 Complaint related: No Description: Based on record review, licensed center did not have the original criminal history record check for staff within 30 days of employment. Evidence: 1. Staff # 1 was hired on 9/5/17. 2. Staff # 1's record did not have the original criminal record check (CRC) which was required before fingerprinting went into effect. Staff # 1's record had fingerprinting results from another agency that are not intended for the purpose of licensing and are not acceptable for licensing. 3. The center did not submit and obtain the CRC for Staff # 1 within 30 days of employment. Plan of Correction: We will send staff for fingerprinting through the appropriate agency.
