22VAC40-191 Background Checks (22VAC40-191)

The licensing inspector conducted an unannounced monitoring inspection on 07/12/2018 from 9:33am to 2:41pm. The children were observed playing with toys, playing on the playground, singing songs, and swimming. A formal observation was completed with the two year old classroom. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today. Ten children?s records and ten employee records were reviewed during this inspection. Medication is not being administered. The center?s injury prevention plan was updated 06/18/2018. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 07/03/2018 Last shelter-in-place drill: 02/6/2018 Last fire inspection: 08/9/2017 Last health inspection: 06/26/2018 Today, the following child to staff ratios were observed: Twos - 13:2 Threes - 10:1 Fours ? 14:2 Fours - 19:2 Fives - 10:2 Kindergartners - 16:1 Young School Age ? 10:1 School Age - 16:1 The violations from the previous inspections were checked for corrections. Description: Based on observation, the center did not ensure that areas and equipment of the center, inside and outside, be maintained in a clean, safe and operable condition. Evidence: 1. The five year old classroom had a 6 foot book shelf which was unattached to the wall, this presented the possibility of the equipment falling on a child if pulled. 2. The school age classroom had a television located on a mounted shelf in the top right corner of the room. A DVD player was located on a lower shelf. Both devices were connected to each other, and the cords were all plugged into an outlet on the wall. Two cords were hanging within reach of children and were not secured to the wall, presenting an entanglement hazard and the possibility of the equipment falling on a child if pulled.

Plan of Correction: The shelf will be secured before business hours tomorrow and the t.v. was removed immediately.

Description: Based on record review, the center did not ensure that one of ten staff records contained required documentation of fingerprint check results before the first day of employment. Evidence: 1. The record of staff #1 (start date: 6/18/18) contained documentation of required fingerprint check results dated 6/20/18. Fingerprint check results must be in each staff members record before employment.

Plan of Correction: We will not start staff before fingerprint check results are received.
