Technical Assistance:
Discussion of the follow-up for the out of state CPS checks for Staff #3 (Pennsylvania), and for Staff #5 (Oregon). It was indicated that both of these requests have been sent to their applicable state. In addition, a CPS check for Staff #2 (Virginia) was re-sent. All noted documentation will be reviewed at the next inspection. Staff #3 indicated that the finger print checks for all current employees have been conducted. As a reminder, all finger print checks for existing employees and board officers must be completed by September 30, 2018. New employees will not be allowed to start until an ?Eligibility Letter? is on file.

Comments:
The following inspection took place between the hours of 10 am through 1 pm. Today, I met with the Staff of Villa Montessori Preschool located in the town of Leesburg, Virginia. At the start of this unannounced monitoring inspection there were a total of 40 children with 8 classroom staff; good ratios in all 4 rooms. A total of 5 children?s files and 5 staff files were reviewed. The children were engaged in the following: music, dance, learning numbers within classroom centers, large motor skilled activities outside on the center playground, art, snack, lunch, nap time, and other organized activities. Standard #: 22VAC40-191-60-D-1
Description: Based on review, 1 of 5 staff files reviewed indicated that a follow-up was not conducted for a requested report that was sent, but not returned within 30 days. Evidence - 1. On the date of inspection (07/05/2018), it was determined that a request for a search of the central registry of Virginia was not available for review. 2. Based on the documentation within Staff #5's file, this request was sent on 05/25/2018. 3. No further information was documented stating that a follow-up call was made inquiring the status of this request during the four working day period after these 30 days.

Plan of Correction: A follow-up call shall be made to inquire the status of this request. If needed, a new request shall be sent immediately.
