Comments:
The licensing inspector conducted an unannounced complaint inspection on 07/05/18 in response to a complaint that was received in the licensing office on 06/27/18 regarding allegations in the area of programs. Interviews were conducted with staff , records were reviewed and observations of the program were made relating to the allegation. The information gathered during the investigation supports the allegations, so the complaint is determined to be VALID. A monitoring inspection was also conducted and other violations were cited, not related to the allegations.

Complaint related: Yes
Description: Based on observation and interview the center did not ensure rough handling of a child was not used. Evidence: On 02/09/18 at 7:10am 2 year-old children #1 and #2 were dropped off by a parent. Staff #3 was observed on the center's security camera rough handling both children. The video shows child #1 and #2 in their coats and crying at a table. Staff #3 stood over the table and spoke to the two children. The written record of counseling used by the center to document the incident described staff #1's voice tone as "harsh and abrasive." The video shows Staff #3 grab child #1's right arm and roughly pull her from the table with the chair dragging beneath her she is seen taking the child to the carpet and removes her coat. Staff #3 talks with child #1 and is seen pointing in the child's face. Child #1 gets up and sits back in the chair. Staff #3 grabs her left hand, removing her from the chair and takes her outside the classroom door. In an interview with staff #4 she stated Staff #3 told her she took child #1 to the door to show her parent had left. Staff #3 walks over to child #2 who is still crying and sucking her thumb, pushes the chair to face her and roughly takes her coat off. Staff #3 grabs child #2's left hand and takes her outside the classroom door. In an interview with staff #4 she stated Staff #3 told her she took child #1 to the door to show her parent had left.

Plan of Correction: The following corrective action was provided by the program director during the exit interview: An emergency staff meeting was held the evening of the incident to address all staff. A written warning was given to staff #3 in regards to the incident. The staff terminated for another infraction of the center policies. In the future, any rough handling will result in immediate suspension and possible termination.

Description: Based on a record review the center did not maintain a record of injuries with all required information. Evidence: Two injury reports dated 03/21/18 and 05/02/18 did not have a second signature.

Plan of Correction: The following corrective action was provided by the program director during the exit interview on 07/16/18: Administration will ensure 2 signatures will be on every injury report moving forward.
